hyperlink from select form with method get to html (php,mysql)

Question

i need to link a select form to "country_data.php" from "index.php" by GET method's. i'm using php_mysqli connection importing a countries list into index.php form. but i wish to set a hyperlink to each row (country) of the list and send the Code(country.table)TO "country_data.php" and show Name(country) in the index.php list. my code try to list-import countries but LINK doesnt work, niether _GET, i appreciate it!
