Trying to populate Excel sheet using two values from another sheet

Question

I am attempting to do a compound lookup for the first image using the values of Column D and E to populate the corresponding value of Column C into the Action Priority Matrix sheet (same workbook). I don't know if this is possible, my current research is leading to me that it could, be, but I've been unable to find an exact solution to this problem.

Any help is appreciated
