matlab - Active Band-Pass filter Simulink design

Question

I need help regarding Simulink, we were supposed to design an active band-pass filter with one low pass filter, one high pass filter, and one voltage follower with the given frequencies fL=3Hz and fH=5Hz, you can assume R, C, and L values then simulate the performance of this circuit with Vin = Sin(2pi0.1t)+Sin(2pi10t)+Sin(2pi*4t). But could not figure out how to write this v-input.

Below is the circuit I designed but I didn't get the correct output.
